This is ponification of Proton M rocket crash in 2013 (of course there was no Falcon there irl, but I decided to add some company for poor Proton)
Due to faulty orientation sensor Proton rocket (one to the right) failed after several seconds in flight, flipping around and falling back on the ground. Investigation found out that sensor was installed the other way around, so orientation system was getting its data flipped.
